Do you have a referral letter ?

All appointments* across Mater Private Network, including consultant consultations, require a referral letter from your GP which must be uploaded with this form. If a GP referral is not included with your request for an appointment, we cannot process your request.

*There are some exceptions - the following services do not require a GP referral letter: Aeromedical, Cosmetic Surgery, Eye Laser, HealthCheck, Menopause Clinic, Physiotherapy and Visa Medical.
