Referrals can be made by staff, patients or family members by arranging for the completion of a referral form available on the ward. If you or your loved one would like to meet with the medical social worker during your hospital stay please let the nurse looking after you know and they will contact us.

About our services

Our department is open from 9am to 5pm, Monday to Friday. We offer the following services:  

  • Psychosocial assessments and psychosocial support  
  • Short term counselling 
  • Information and advocacy regarding local community support services for patient’s daily aftercare needs
  • Linking of patients to services that will enhance their ability to stay well and prevent  readmission
  • Assistance to patients and their families with discharge from the hospital setting  
  • Support and education to patients’ families to better equip them to handle their role as caregivers
  • Short-term counselling, education, and psychotherapy referrals for oncology patients  
  • Information and advice regarding long term care, convalescence care, Fairdeal applications, etc
