Preparing for Discharge

You will be advised of an estimated or planned discharge date but your Consultant will finalise your discharge.  The main thing to have arranged in advance is someone to collect and accompany you home.

Before your admission, chat with your Consultant about how you will be day-to-day following your procedure or stay. This will help you plan ahead for any extra support you might need once you get home or indeed, it will help you decide if you might benefit from some respite care. If you need any additional support and have difficulty in organising it, ask to talk with a member of our social work team who will work with you to put plans in place.

You will be advised of an estimated or planned discharge date to help you plan for your return home but your Consultant will finalise your discharge.

Where possible you should be collected by a family member or friend on discharge. If you would like to travel by taxi the ward staff will be happy to help arrange this for you
